Clinical Social Work Consultant – Care Coordination

OhioHealth is seeking a qualified Social Worker to support biopsychosocial assessments, treatment planning, and discharge coordination at the UM Care Coord-North Hub. The role involves working with an interdisciplinary team to provide patient-centered care and ensure appropriate resource linkage.

Master’s in Social Work and LSW license are required, with 1–2 years of related experience preferred. Day shift; on-site position with flexible hours as needed.
