Community Mental Health Practitioner – Home-Based Care & Recovery

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting Mental Health Practitioners to deliver intensive, home-based treatment as an alternative to hospital admission. You will support patients and families at critical points in their care journey, helping reduce inpatient stays and promote recovery and independence in the community.

The role requires registration as an MH professional (RMN, Social Worker, OT) and the ability to travel across Winchester, Basingstoke, and Havant.
