Lead Community Nurse – Home Visits & Case Management

Gloucestershire Health and Care NHS Foundation Trust is recruiting for a Part-time Community Nurse lead with 0.7 WTE to manage a Band 5 team and deliver home-based care across Gloucestershire. The role requires a driving licence for travel to patients’ homes and extensive community nursing experience.

You will work within the Integrated Care System, supporting high-quality, patient-centred care and leading improvements in service delivery with access to training and professional development.
