Lead Advanced Practitioner – Adult Social Care

Cheshire East Council is seeking an Advanced Practitioner to lead the Adult Social Care Community Team. You will work with the Practice Manager to drive high-quality, person-centred practice, support practitioners across teams, and champion continuous improvement through supervision and development.

You will oversee complex cases, mentor social workers and students, and collaborate with partner agencies to deliver better outcomes for residents.
