Frailty GP Leader — Home Visits & MDT Care

DORKING HEALTHCARE LIMITED is seeking a Frailty General Practitioner to lead the PCN frailty hub and deliver high-quality, home-based care within a mature collaborative network.

The post holder will coordinate with Community Matrons, District Nurses, Pharmacists and other providers to proactively triage frail patients, develop care plans, and reduce unplanned hospital admissions through MDT approaches across the local health system.
