PCN Social Worker – Adult Social Care & Community Support

Wokingham Borough Council is seeking a qualified Social Worker to join the Primary Care Networks team within Adult Social Care on a full-time permanent basis. You will provide social work expertise to patients with complex social and safeguarding needs in primary care settings.

You will collaborate with GPs, social prescribers and voluntary sector partners to prevent escalation and promote independence, with strong emphasis on person-centred care and safeguarding.
