START Care Coordinator: I/DD & Behavioral Health

YAI seeks a START Coordinator to manage a caseload of individuals with intellectual/developmental disabilities and co-occurring behavioral health needs. You will conduct evaluations, provide resources, and coordinate with an interdisciplinary team to support crises and discharge planning.

The role requires on-call coverage and regular outreach visits, with travel across regions as needed. A Master’s degree in a related field is required along with START Coordinator certification within 12 months.
