Discharge Co-ordinator – 7‑Day Transfer of Care Hub

East Sussex Healthcare NHS Trust is seeking Discharge Co-ordinators to join the Transfer of Care Hub. The role supports safe, timely discharges from acute hospital settings and works with multi‑agency teams across Conquest and Eastbourne Hospital.
