Community Nurse: Home-Based Care & Teamwork

Cornwall Partnership NHS Foundation Trust is seeking a Community Nurse for the Hayle area (Hayle, St Ives and Marazion). The role involves assessing, planning, implementing and evaluating care in patients’ homes, with early induction and ongoing clinical supervision.
