HCRG Care Group is seeking a Bank Advanced Clinical Practitioner to deliver acute hospital-level care at home across Wiltshire, with responsibility for assessment, diagnosis and management of complex cases. You will work within a multidisciplinary team, ensuring safe discharge and timely review using remote monitoring.
Travelling across Wiltshire, you will provide leadership and develop care plans while upholding high clinical standards and safety protocols in line with NMC/HCPC guidelines.
