Leaving Care: Advanced Social Work Practitioner (Locum)

Sanctuary Personnel Ltd in Stoke-on-Trent is seeking an Advanced Social Work Practitioner for the Leaving Care Next Steps Team on a three‑month locum basis, at a competitive rate of £33.30 per hour.

You will assess young people leaving care, develop tailored pathway plans for independent living, collaborate with multidisciplinary partners, and provide guidance to social work colleagues to promote best practice.
