Hospital-at-Home ACP: Lead Acute Care in Bath

HCRG Care Group invites applications for an Advanced Clinical Practitioner (ACP) based in Bath as part of our Hospital at Home service. You will deliver acute, hospital-level care at home across the locality, supporting step-up and step-down pathways and reducing unnecessary hospital stays.

The role requires NMC/HCPC registration, an Accredited Advanced Clinical Practice qualification, and non-medical prescribing, with leadership experience in multidisciplinary teams and a clean driving licence.
