Job Description:
St Luke’s Hospice Plymouth is seeking a Band 5 Nurse/Paramedic to specialise in end-of-life care within the End-of-Life Urgent Care Service. You will support community-based EOL patients in achieving their preferred place of care and death while preventing unnecessary hospital admissions.
The role requires autonomous working with comprehensive holistic assessments and planning, addressing practical, psychological, spiritual, and social needs.
