Transitions Lead for Young Adults (Social Care)

Lancashire County Council in Lancashire invites applications for a Transitions Manager within Adult Social Care. You will lead the Young Adults Service team, supporting young adults aged 18–25 through key life transitions to promote independence and positive outcomes.

The role provides professional oversight of Social Workers and staff, oversees safeguarding, assessments and support planning, and collaborates with education, health and community partners to enable access to opportunities.
