Queen Alexandra Hospital in Portsmouth is seeking an innovative Advanced Clinical Practitioner to join the Mental Health Liaison Team. The role supports adults over 18 with acute mental health concerns or deterioration of existing conditions, in a 24/7 service.
You will perform holistic assessments, develop recovery-focused care plans, and provide specialist assessments. Collaborate with MDT staff and liaise with multi-agency colleagues; contribute to education and training for clinical staff.
