We have an exciting opportunity for a full-time Patient Pathway Coordinator to join the Community CAMHS Neurodevelopmental Pathway at Sheffield Children’s NHS Foundation Trust.
This is a pivotal role within a busy and evolving service that supports children and young people referred for assessment and support relating to neurodevelopmental needs, including Autism and ADHD pathways. As demand for neurodevelopmental services continues to grow, the Patient Pathway Coordinator will be instrumental in ensuring children and young people experience a well-coordinated, efficient and compassionate journey through the service.
Working closely with clinicians, operational managers and administrative colleagues, you will coordinate referrals, manage patient pathways, monitor waiting lists and support service performance, helping to ensure children and families receive timely access to care.
Main duties of the job
Your responsibilities will include:
- Coordinating referrals and patient pathways through the Integrated Front Door.
- Monitoring waiting lists and supporting pathway performance against agreed standards.
- Managing appointments, bookings and communications with families and professionals.
- Working closely with multidisciplinary teams to support effective service delivery.
- Maintaining accurate patient records and ensuring high standards of data quality.
- Producing and monitoring activity and performance information.
- Supporting service improvement initiatives as part of the wider Mental Health Transformation Programme.

Person Specification
Qualifications and Training
- NVQ Level 4 or equivalent experience
- 5 GCSEs or Equivalent (grades C/4 or above)
- IT qualification or evidence of advanced administrative training
Experience
- Working knowledge of hospital systems (e.g. PAS/SystmOne)
- Experience of patient tracking, monitoring waiting lists and performance targets
- Experience of managing information, sharing and analysing data
- Previous experience of developing and implementing new office systems
Knowledge and skills
- Understanding of confidentiality and data protection requirements
- Evidence of excellent organisational skills, prioritisation and communication skills
- Experience of being able to work as part of a team, whilst being able to work independently
- Comprehensive knowledge of Microsoft Office and ability to use Excel spreadsheets and pivot tables
- Ability to use information to effectively support the improvement of performance and service delivery
- Awareness of safeguarding responsibilities
