Community LD Nurse

We are seeking a dedicated and compassionate Community Learning Disability Nurse to join our multidisciplinary team. This is an exciting opportunity to make a real difference in the lives of people with learning disabilities, supporting them to achieve better health outcomes and live fulfilling lives in the community.
Role:
- The assessment, planning, implementation and evaluation of programmes of care for children and adolescents who have learning disabilities and mental health difficulties.
- Managing a caseload under supervision, offering a range of clinical interventions to meet the mental health needs of children and adolescents with learning disabilities.
- Work collaboratively with families, carers, and other professionals across health and social care.
- Providing supervision, where agreed, to more junior members of the team.
Requirements
- Registered Nurse (RNLD).
- 12 months post-registration experience, learning and professional development.
- Experience working with people with learning disabilities in community settings.
- Strong communication, leadership, and problem-solving skills.
- Ability to work autonomously and as part of a team.
Main duties of the job
- To provide specialist support, advice and guidance to individuals, families and carers.
- To provide specialist support as part of the multi-disciplinary team
- To complete specialist assessments as clinically indicated.
- To participate in PBS training and offer specialist support within the PBS framework.
- To participate in referral meeting, team meetings and wider MDT meetings.
- To contribute to team and service development.
- To carry an individual caseload.
- To keep up to date with research and development in your fields of practice.
- To attend training relevant to the role.
- To maintain NMC registration and remain up to date with research and development in learning disability nursing practice.
- To complete individual risk assessments under supervision and to put a safety plan in place if needed.
- To communicate with and support social workers, care providers and other agencies and services.
- To supervise and assess student nurses on placement with the team.

Job responsibilities
- 1. To practice within the NMC Code of Professional Conduct.
- 2. To be professionally accountable for the effective and efficient management of a defined caseload, taking into account the need to combine your own responsibilities and those of the multi-disciplinary team.
- 3. To be responsible for setting high standards of care within the resources available and to review and monitor these on a continuous basis through the process of audit, case review and on-going supervision.
- 4. To promote a positive approach to health education and health promotion.
- 5. Participate in consultancy and specialist input to a range of other professionals and agencies in relation to individuals with complex needs.
- 6. Advise and influence local and regional policy, strategy and operational practice in relation to health needs of people with learning disabilities.
Person Specification
Essential and desirable
- Evidence of working in a multi-disciplinary/agency setting
- Experience of assessing and managing mental and physical health conditions
