Paramedic, Advanced Nurse or Clinical Practitioner
An exciting opportunity has arisen to join East Norfolk Medical Practice’s (RCGP) award-winning Urgent Care Team (UCT).
We are looking for a motivated and enthusiastic Paramedic, Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to become part of a supportive, innovative and clinically focused team within one of the largest GP practices in Norfolk.
Our Urgent Care Team was established in 2018 and works alongside our team of 20 GPs (soon to be 22), providing both routine and urgent care to patients. Unlike many urgent care models, our team is not designed to replace GPs but to work in partnership with them, ensuring patients continue to benefit from continuity of care and easy access to clinical expertise.

Main duties of the job
The successful applicant will work as part of both the Practice’s routine primary care service and the Urgent Care Team.
Typically, mornings are spent working alongside an allocated GP, supporting the care of their personal patient list and helping deliver continuity of care. Afternoons are spent within the Urgent Care Team assessing and managing patients requiring same-day care, supported by two Duty GPs.
The role includes:
- Assessment, diagnosis and treatment of patients presenting with acute and chronic conditions
- Face-to-face and telephone consultations
- Requesting and interpreting investigations
- Developing and implementing management plans
- Reviewing and actioning clinical results and correspondence
- Undertaking home visits and care home visits when required
- Referring patients appropriately within primary, community and secondary care services
- Working collaboratively within a multidisciplinary team to deliver high-quality patient care
About us
East Norfolk Medical Practice is a large, forward-thinkingpractice serving more than 32,000 patients across four sites.
Our diverse patient population ranges from urbancommunities, including one of the most deprived wards in the country, to a growing coastal village. We also hold the contract to provide enhanced healthcareservices for the area’s homeless population.
We currently have 20 GPs, including 15 GP Partners, with twoadditional GPs joining in the coming months. This provides excellent clinicalsupport and supervision for members of the Urgent Care Team.

Job responsibilities
The post holder will work within their professional scope of practice to provide high-quality clinical care for patients registered with East Norfolk Medical Practice.
They will work closely with GPs and the wider multidisciplinary team to assess, diagnose, treat and manage a wide variety of patient presentations, supporting both routine primary care and urgent care services.
Clinical Responsibilities
- Assess, diagnose and manage patients presenting with acute and chronic conditions.
- Utilise advanced clinical assessment and decision-making skills.
- Conduct face-to-face and telephone consultations.
- Request and interpret investigations including blood tests, imaging and ECGs.
- Develop, initiate and review patient management plans.
- Review and action clinical results, referrals and correspondence.
- Undertake home visits and care home visits as required.
- Refer patients appropriately to primary, community and secondary care services.
- Work within local and national guidelines and clinical protocols.
- Recognise limitations of own knowledge and seek support when appropriate.

Clinical skills & knowledge

Experience

Qualifications
- Qualified Paramedic, Advanced Nurse Practitioner or Advanced Clinical Practitioner
- Current registration with the appropriate professional body
