Senior Hospital Case Worker – Discharge & Care Planning

Hampshire County Council is seeking a Senior Case Worker (Hospitals) to join our Hospitals team in Basingstoke. This permanent, hybrid role involves completing assessments of care and support needs across hospital, community hospitals and care homes, with a focus on enabling wellbeing and independence.
