Working within Eden’s GP practices and working closely with the wider ICC team, the Living Well Coach (LWC) will work within the Wellbeing Service and target and provide assessment for individuals with LTC, Frailty and psycho-social needs engaging them to set personalised goals and find and access services and resources best suited to meet their needs and circumstances.

Depending on the individual the assessment might include basic health assessment, low level clinical screening, wellbeing assessment, review of functional abilities, assessment of social / housing / employment circumstances and gaining understanding of social and support networks.

LWCs will develop skills in active listening, motivational interviewing and appropriate psychological care skills including CBT, acceptance & commitment therapy, mindfulness. Training and on-going clinical supervision will be provided by the Physical Health Psychology Department within North Cumbria Integrated Care NHS Foundation Trust (NCIC).

Main duties of the job

Using their coaching skills to support individuals identified by the GP practice and wider Integrated Care Community (ICC), Living Well Coaches (LWC) work alongside people who want to improve their skills in self management of their own health and wellbeing, the coach will listen to what is important to the person and help them to identify and commit to step by step activity , building towards their improvement goal.

Although the base will be within one/ two GP practices in the Eden Integrated Care Community (ICC) the post-holder will be expected to work in a range of environments including visiting people in their own homes, in other GP practices or community health/ care settings. Individual LWCs will be allocated to one or two practice(s) allowing continuity for patients and direct engagement with the GP practice team.

Qualifications

  • A qualification in Health / Social Care or an accredited award such as NVQ Level 3 or prior work experience of similar work
  • Care Certificate or the willingness and ability to undertake (applicable for all non-registered support workers that provide health care)
  • ECDL or equivalent

Experience

  • Experience of working within a team
  • Experience of organising own day to day work and adjusting priorities in response to changing needs or needs of others
  • Experience of lone working and/or understanding of risks associated with this
  • Experience of working in a health or social care setting
  • Experience of delivering person centred care

Knowledge

  • Understanding of the needs of vulnerable people
  • Able to recognise where individuals might be at risk and take appropriate action
  • Knowledge of Health & Safety procedures and practices to be considered when working with clients and cares in a variety of settings
  • Core competencies in use of MS office and in data entry as there will be a need to enter into, GP Practice systems, including creating, updating, and amending, care plans and assessments
  • An understanding of and commitment to policies in relation to Equal Opportunity, Customer Care and service delivery and data sharing, and the ability to implement these policies in the workplace
  • Experience of using EMIS and PRIMIS information systems
  • Working knowledge of the range of psychological and physical health problems that affect adults
  • Working knowledge of relevant legislation such as safeguarding, vulnerable adults and mental capacity and how these impact on casework
  • An understanding of the resources available in the locality to help and support both individuals and their carers or where this information can be found
