Discharge to Assess Social Worker – Hybrid, Nottingham

Sanctuary Personnel Ltd in Nottingham is seeking a Social Worker to join the Discharge to Assess Team on an ongoing locum basis. The role supports adults through hospital discharge pathways, conducting D2A assessments and contributing to CHC processes with person-centred care planning.

This role offers full-time hours (37 per week) and a hybrid working arrangement to enable flexible, high-quality discharge coordination and collaboration with health and social care colleagues.
