What you will do
- Deliver expert pharmaceutical care and optimise medicine use across multiple specialties.
- Undertake medicines reconciliation and screen prescriptions to ensure safety and efficacy.
- Support safe and cost-effective prescribing and facilitate timely discharges.
- Ensure a seamless transfer of care across sectors.
- Contribute to clinical governance and the development of guidelines.
- Supervise and mentor trainees and Band 6 pharmacists.
- Participate in audits and quality improvement initiatives.
- Collaborate with rotation leads to enhance pharmacy services.
Requirements
- HCPC registration is required.
- Ability to drive to ensure you can easily reach patients across different settings.
