Senior Prescribing Clinician – Urgent Care & Care Coordination

HealthHero is seeking an independent Prescribing Clinician to support Care Coordination and Integrated Urgent Care services across Bath, Swindon, and Wiltshire. You will provide clinical assessments and system navigation to support patient care and reduce hospital admissions.

This position allows for both part-time and full-time hours and involves working across various clinical settings, including face-to-face and remote consultations.

The ideal candidate will have strong clinical reasoning, triage skills, and a non-medical prescribing qualification along with experience in urgent care.
