Devon County Council is seeking a Social Worker to support adults through transitions from 17+ into adult services and into older age. You will assess needs under the Care Act, support decision-making per the Mental Capacity Act, and safeguard adults at risk.
You will work with service users, families, care providers and health professionals to empower individuals to live independently and safely in their communities.
