What you will do:

  • Undertake statutory reviews of care and support plans in accordance with the Care Act 2014, ensuring services remain appropriate and outcome-focused.
  • Complete social care assessments for adults and carers, including reassessment of needs where circumstances have changed.
  • Carry out Mental Capacity Assessments and contribute to Best Interests decisions in line with the Mental Capacity Act 2005.
  • Identify and respond to safeguarding concerns, progressing issues appropriately in line with local procedures.
  • Develop and review care and support plans, ensuring a strengths-based and person-centred approach.

Person Specification:

Qualifications

  • Hold a degree in Social Work or an equivalent recognised qualification.
  • Registered with Social Work England.

Registration & Compliance

  • Possess knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
  • Understand CQC standards and local authority statutory duties.
  • Enhanced DBS clearance is required, ensuring you meet the highest standards of professional practice.
