Senior Practitioner – Adult Social Care (Complex Care & Therapy)

Location: London, Greater London

Pay Rate: £41.95 per hour (Umbrella)

Contract: Full-Time | Agency

The Role

As a Senior Practitioner, you will provide expert assessment and advice for adults with complex care and therapy needs, ensuring services are effective, consistent and focused on achieving meaningful outcomes. You will work closely with a multidisciplinary team, including Occupational Therapists, Sensory Impairment Practitioners and Equipment Managers, supporting the delivery of person‑centred care and promoting independence. You will also support the Team Manager with leadership responsibilities and deputise when required.

Key Responsibilities

  • Complete complex assessments and provide specialist advice across Adult Social Care services.
  • Make robust, evidence‑based decisions regarding care pathways, support planning and risk management.
  • Support adults, carers and families to achieve positive, personalised outcomes.
  • Act as a recognised specialist within your area of expertise.
  • Provide guidance and support to colleagues to promote best practice.
  • Work collaboratively within a multidisciplinary team environment.
  • Support the Team Manager in leading a team of Occupational Therapists, Sensory Impairment Practitioners and Equipment colleagues.
  • Deputise for the Team Manager when required.
  • Contribute to the development of practice, procedures and service improvements.
  • Promote strengths‑based approaches, independence and choice for service users.
  • Work effectively with health professionals, partner agencies and community services.

About You

  • Significant experience within Adult Social Care, Occupational Therapy, Social Work or a related professional field.
  • Strong experience managing complex care needs and risk assessments.
  • Excellent knowledge of adult safeguarding, care planning and statutory responsibilities.
  • Experience working within a multidisciplinary team.
  • Ability to provide expert advice and support complex decision‑making.
  • Strong communication, analytical and problem‑solving skills.
  • Experience contributing to service development and improving practice.
  • A commitment to delivering person‑centred, outcome‑focused services.
