Job Description:
We Do Social Work is recruiting a Mental Health Social Worker to join a Hospital Discharge Team in the North West. You will support discharge from psychiatric inpatient units, coordinating move‑on arrangements and liaising with NHS colleagues to ensure safe, person‑centred outcomes.
You will operate in a busy environment with strong knowledge of the Care Act, Mental Health Act and related legislation, ensuring accurate records and excellent risk management. Hybrid working is offered.
