Locum Social Worker within AdultExtra Care / Housing Transition Team – Bromley, £32 Hourly. What you will do:

  • Conduct timely and proportionate Care Act needs assessments and reviews for residents of the Extra Care Housing scheme.
  • Guide residents through the transition from an Extra Care model, ensuring their care plans are updated to reflect revised accommodation and service arrangements.
  • Collaborate with the housing provider, outgoing and incoming care providers, commissioners, and health professionals to ensure continuity of care.
  • Identify and mitigate risks associated with changes to care provision and accommodation, implementing appropriate risk management and safeguarding measures.
  • Carry out Mental Capacity Assessments and contribute to Best Interest decision-making when required.
  • Ensure residents, their families, and advocates are kept informed and supported throughout the transition process.
  • Maintain precise, timely, and defensible case recording in adherence to statutory and organisational requirements.
  • You must be a Qualified Social Worker with current registration with Social Work England.
  • Recent experience in adult social care is essential, ideally with a focus on older persons and/or housing-based support services.
  • A robust understanding of the Care Act 2014, the Mental Capacity Act 2005, and safeguarding duties is required.
  • Proven experience in conducting assessments and reviews within Extra Care, supported housing, or similar settings.
  • Experience supporting service transitions or provider changes is highly advantageous.
  • Previous involvement with partnership working with housing providers is beneficial.
  • Knowledge of commissioning and care provider handover processes is a plus.
