Senior Practice Leader — Adult Social Care West Review

Lincolnshire County Council is recruiting an Advanced Practitioner – West Review Team within the West Adult Community Wellbeing Team. You will lead advanced practice, coach colleagues, and manage a complex caseload for adults across Lincolnshire.

You will work with health partners and community organisations to deliver integrated support, deliver safeguarding expertise, and mentor staff while promoting strengths‑based, person‑centred care.
