Hospital Discharge Social Worker — Community Care

Devon County Council is seeking Social Workers to join the discharge to assess team, supporting people to recover after hospital stays and plan their return home. You will work in MDTs, completing Care Act assessments and care plans, coordinating ongoing social support and safeguarding information when needed.

This role offers hybrid working, a strong development path, and a commitment to inclusive practice. A SWE registration and DBS are required.
