Part-Time Social Prescriber: Community Care Outreach

Bournemouth East Collaborative PCN is seeking an experienced Social Prescriber to join our Mental Health Team for 20 hours per week at Southbourne Surgery, Beaufort Road, BH6 5BF.

You will work within a multidisciplinary Primary Care Network to support patients with non-medical needs, focusing on vulnerable groups and improving access to local services. DBS clearance is required or activated via DBS service. This role involves community engagement and partnership development.
