What You’ll Be Doing:

• Managing a caseload of adults with complex care and support needs
• Completing comprehensive assessments and developing personalised care plans
• Reviewing existing care packages and making recommendations to ensure support remains effective
• Working closely with individuals, families, carers, and external professionals to achieve positive outcomes
• Collaborating with multidisciplinary teams to coordinate safe and effective care
• Promoting independence, choice, and person-centred approaches in your practice
• Supporting safeguarding processes and ensuring statutory responsibilities are met

What We’re Looking For:

• Qualified Social Worker
• Current registration with Social Work England (SWE)
• Degree/Diploma in Social WorkExperience working within Adult Social Care, Complex Care, or a similar service area
• Strong assessment, care planning, and review experience
• Knowledge of relevant legislation, including safeguarding responsibilities
• Evidence of continued professional development
• Full UK driving licence
