Preparing for Adulthood Practitioner (Social Care 14–25)

Hertfordshire County Council is recruiting a Preparing for Adulthood Practitioner to support young people aged 14–25 and their families. You will manage a caseload, conduct needs assessments, and review care packages in collaboration with schools, health professionals, and SEND teams.

The role requires a social work qualification, good communication, and the ability to work under pressure; a full driving licence is essential. Base locations are Apsley or Stevenage with county-wide travel.
