Job Summary
The Welby Group is seeking an experienced Adult Mental Health Practitioner to join our expanding Mental Health Team.
This is an exciting opportunity for an autonomous practitioner with excellent assessment, triage and clinical decision-making skills who is passionate about improving access to high-quality mental health care within primary care.
Based within our Central Hub in Bottesford, you will provide predominantly remote consultations to adults presenting with a broad range of mental health conditions. You will work alongside our GPs, Mental Health Support Worker and wider multidisciplinary team to deliver timely assessments, personalised treatment plans and onward referrals where appropriate.
We are particularly interested in applicants who hold an independent prescribing qualification or are working towards one.
Main duties of the job
The successful applicant will:
- Undertake comprehensive mental health assessments for adults presenting with a wide range of mental health concerns.
- Deliver safe, effective remote consultations using a holistic and patient-centred approach.
- Provide prescribing within your professional scope of practice (where qualified).
- Assess and triage patients, ensuring appropriate prioritisation and escalation where required.
- Support patients through ADHD and Autism referral pathways as part of a wider mental health caseload.
- Develop and review personalised care plans, providing ongoing follow-up where appropriate.
- Work collaboratively with GPs, the Mental Health Support Worker and external mental health services.
- Promote prevention, early intervention and self-management strategies.
- Maintain accurate clinical records and coding using the practice clinical system.
- Participate in clinical governance, audits and quality improvement initiatives.
- Contribute to the ongoing development of mental health services across The Welby Group.
- Provide clinical support and guidance to the Mental Health Support Worker.

The Adult Mental Health Practitioner will provide specialist mental health assessment, support and treatment to adults registered with The Welby Group. Working as an autonomous practitioner within a primary care setting, the post holder will provide high-quality, evidence-based mental health interventions and contribute to improving access to mental health support for our patient population.
The post holder will work as part of a multidisciplinary team, collaborating closely with GPs, healthcare professionals, and external mental health services to ensure patients receive timely, appropriate and coordinated care.
The role will involve undertaking comprehensive mental health assessments, supporting patients with a range of presentations including common mental health conditions, complex needs, crisis presentations, ADHD and Autism-related concerns, and developing personalised care plans that support recovery, self-management and improved wellbeing.
The successful applicant will work autonomously within their professional scope of practice, using advanced clinical skills to assess, triage, diagnose where appropriate, prescribe (where qualified) and make clinical decisions regarding patient care pathways.
The post holder will also contribute to the ongoing development of mental health services across The Welby Group, supporting quality improvement, clinical governance and the delivery of innovative approaches to patient care.
Key Areas of Responsibility
Clinical Practice
- Provide specialist mental health assessment, advice and support to adults presenting with a range of mental health concerns.
- Undertake clinical assessments, including risk assessments, formulation and care planning.
- Provide evidence-based interventions and support patients in managing their mental health.
- Prescribe safely and effectively within scope of practice where appropriately qualified.
- Identify when escalation, safeguarding input or onward referral is required.
- Support patients accessing ADHD and Autism assessment pathways.
- Provide follow-up and review appointments to monitor progress and outcomes.
