Primary Healthcare Darlington is seeking an experienced Care Co-ordinator to join the Enhanced Health in Care Homes team in Darlington. The role focuses on coordinating care planning and support for residents in CQC registered care homes and organising MDT meetings with NHS professionals.
You will liaise with GPs, practice staff, community nurses and other partners, ensuring care is aligned to residents’ needs and information is clearly communicated.
