Social Worker – Primary Care Network (Adult Social Care)

Wokingham Borough Council is recruiting a Social Worker to join the Primary Care Networks in Adult Social Care. This full-time, permanent role focuses on enabling people to live the lives they choose, working with GPs, social prescribers, and voluntary sector partners to prevent needs from escalating.

You will be based across GP practices within a PCN, supporting individuals with complex social and safeguarding needs, and contributing to integrated community-based care. DBS enhanced checks apply.
