The Role
To lead, manage, develop and evaluate the pharmacy technician input to the clinical pharmacy services at base hospital alongside the Principal Pharmacist – Clinical Services (and other senior pharmacists) to ensure safe, effective and person-centred pharmaceutical care that complies with relevant local and national guidelines, legislative requirements and professional standards as outlined by the General Pharmaceutical Council (GPhC)
To provide specialist professional practice within the acute ward environment, including involvement with the medicines reconciliation process, educating patients on the use of medicines and identification and resolution of pharmaceutical issues, where appropriate
In this key role, you will:
To be involved in the medicines reconciliation process by retrieving, documenting and reconciling information from multiple sources about an individual’s medicines. Any discrepancies to currently prescribed medicines are documented and escalated to the appropriate member of the clinical team.
To contribute to and document within individual patients’ care plans by identifying, assessing and responding to pharmaceutical care issues within own sphere of competence, for example, therapeutic drug monitoring.
To educate patients and carers on the use of medicines and support concordance by providing verbal and written information, including indication, dose, treatment duration, and potential side-effects. For example, utilising anticoagulant patient education checklists.
To assess the quality of patients’ own medicines and evaluate appropriateness for reuse during admission or following discharge, where appropriate.
To assess the appropriateness of multi-compartment compliance aids (MCAs) and liaise with community pharmacies to confirm current arrangements (at both admission and discharge) or establish new arrangements for patients, where appropriate.
What You’ll Bring:
Registered with the General Pharmaceutical Council
SVQ Pharmacy Services Level 3 plus accredited underpinning knowledge National Certificate in Pharmaceutical Sciences
HNC in Pharmaceutical Sciences or Management (or equivalent experience in managing and supervising staff)
Professional Development Award (PDA) – Dispensary Checking Technician Qualification (DCT)
Scottish Vocational Qualification (SVQ) assessor unit(s) (or willingness to work towards)
Continuing professional development as detailed by the General Pharmaceutical Council
