Advanced Social Work Leader — Flexible Community Care

Nottinghamshire County Council is seeking an Advanced Social Work Practitioner to join the Broxtowe Community Social Work Team. You will lead practice improvements, develop all-age working practices, and support managers to drive high-quality adult social care.

You will assess needs, create tailored care plans, engage with service users and families, ensure safeguarding, and promote innovation and continuous improvement across the team, in line with legislation and best practice.
