Royal Free London NHS Foundation Trust is seeking a flexible and experienced Band 7 Physical Health Community Matron to join the CHAT team serving Enfield and parts of Haringey. The post involves managing care home cohorts, coordinating with multidisciplinary teams, and supporting end-of-life planning and Trusted Assessor pathways.
The successful candidate will rotate across allocated care homes and NMUH, with 7 days a week service 8am–8pm and weekend shifts on a rota.
