We have an outstanding opportunity for a Band 5 Community Nurse to join the vibrant Home First South Team within Community General Nursing in Fareham. This role, with the option for Full or Part Time hours, offers a 3 month Block Booking and operating within the hours of 09:00 – 17:00. What you will do:

  • Undertake holistic assessment, planning, implementation, and evaluation of patient care within the Discharge to Assess (D2A) pathway.
  • Deliver evidence-based nursing interventions in patients’ homes, focusing on supporting rehabilitation, recovery, and independence.
  • Facilitate early supported discharge and admission avoidance through proactive care management.
  • Work collaboratively within a multidisciplinary team, liaising with health, social care, and voluntary sector partners to coordinate care.

Qualifications:

  • Registered Nurse (Adult) with an NMC Active PIN
  • Relevant pre-registration nursing degree or diploma

Registration & Compliance:

  • Current registration with the Nursing and Midwifery Council (NMC)
  • Knowledge of NHS clinical governance frameworks and CQC standards
  • Understanding of the Mental Capacity Act and safeguarding principles
  • Commitment to Continuing Professional Development (CPD) and revalidation requirements

Experience:

  • Post-registration experience in a community, acute or integrated care setting
  • Experience supporting patients with complex health needs in the community
  • Exposure to discharge planning or admission avoidance services is desirable
  • Experience working collaboratively across health and social care systems
