We have an outstanding opportunity for a Band 5 Community Nurse to join the vibrant Home First South Team within Community General Nursing in Fareham. This role, with the option for Full or Part Time hours, offers a 3 month Block Booking and operating within the hours of 09:00 – 17:00. What you will do:
- Undertake holistic assessment, planning, implementation, and evaluation of patient care within the Discharge to Assess (D2A) pathway.
- Deliver evidence-based nursing interventions in patients’ homes, focusing on supporting rehabilitation, recovery, and independence.
- Facilitate early supported discharge and admission avoidance through proactive care management.
- Work collaboratively within a multidisciplinary team, liaising with health, social care, and voluntary sector partners to coordinate care.
Qualifications:
- Registered Nurse (Adult) with an NMC Active PIN
- Relevant pre-registration nursing degree or diploma
Registration & Compliance:
- Current registration with the Nursing and Midwifery Council (NMC)
- Knowledge of NHS clinical governance frameworks and CQC standards
- Understanding of the Mental Capacity Act and safeguarding principles
- Commitment to Continuing Professional Development (CPD) and revalidation requirements
Experience:
- Post-registration experience in a community, acute or integrated care setting
- Experience supporting patients with complex health needs in the community
- Exposure to discharge planning or admission avoidance services is desirable
- Experience working collaboratively across health and social care systems
